The Standard 40-Session Framework
The 40-session, 2.0 ATA course delivered five days per week is the research-standard framework for neurological HBOT, emerging from the Efrati Lab trials across TBI, stroke, long COVID, fibromyalgia, and age-related cognitive decline. Over an 8-week period, patients attend one 90-minute session per day Monday through Friday, with weekends off.
Interim Milestones
- Session 10 (approximately week 2): First interim check. Review tolerability, address any ear-equalisation issues, confirm the patient is settling into the routine.
- Session 20 (approximately week 4): Mid-course assessment. For CMS-reimbursed wound-care protocols this is mandatory for continued coverage documentation. For neurological protocols, interim cognitive or functional measures help identify early responders.
- Session 30 (approximately week 6): Late-course check. Many patients notice the clearest clinical improvement starting around this point.
- Session 40 (end of course, approximately week 8): Post-course assessment. Repeat the baseline cognitive, functional, and imaging measures.
- 1 month post-course: Follow-up assessment. Neuroplastic improvement often continues to accumulate for weeks after the course ends.
- 3 months post-course: Second follow-up. Captures the full post-treatment response window.
Why Weekends Off?
The research-standard protocol has weekends off intentionally. Daily seven-days-a-week courses have not been shown to improve outcomes and significantly increase the attendance burden. The weekend break appears to allow physiological recovery between intensive five-day blocks without compromising the cumulative therapeutic effect.
Handling Missed Sessions
Occasional missed sessions are not catastrophic. The protocol tolerates some missed days — most commonly added at the end of the course rather than attempting to double-up within the week. Multiple consecutive missed sessions (particularly early in the course) should be discussed with the hyperbaric team, as the compressed delivery is thought to drive the cumulative response.
Shorter and Longer Variants
While 40 sessions is the research standard, variations exist. Wound-care protocols for diabetic foot ulcers typically run 20–40 sessions depending on healing response. Radiation injury courses often extend to 30–40 sessions. Longevity protocols (Hachmo et al. 2020) use 60 sessions. Choose session count based on the specific indication and physician recommendation.
